The ‘D’ day for issuing the SUS card will be Friday, 27th, at UBS’s in Santarém | Santarem and the region

According to the Municipal Health Service (SMSA), the measure will decentralize the service providing care closer to residents and reach those who have some difficulty in reaching the center that issues the document.

The card can also be issued or updated at any time in the Semsa Building located on Avenida Mendonça Furtado, 2440, in the Aldeia neighborhood. Opening hours are from 8 am to 4 pm.

The SUS card facilitates access to the network of services of the Unified Health System, such as booking appointments and exams, as well as guaranteeing access to free medicines. The document is one of those that must be presented in the process of vaccination against Covid-19, so it is essential that everyone has the card.

For registration, adults must bring the following documents: RG, CPF and proof of residence. For children, certificate, CPF and proof of residency are required. Residents of rivers and plateau areas do not need to provide proof of residence.

Federal government application “Connect SUS” is a virtual platform that does not issue the card, it only allows visualization of the document and vaccinations that patients have taken.
