Those who have already re-enrolled will automatically be included in the list of those eligible for immunization.
Children who do not have a SUS card Salvador Or link registration with another municipality, the parent or guardian must search for any of the units of the vicinity to obtain the document or transfer of residence. The service appointment must be scheduled through Scheduled website.
The documents required of the parents are ID card, CPF, proof of residence and SUS card. For children, it is necessary to present a birth certificate and SUS card (if applicable).
